There is a moment, somewhere around forty, when the conversation with your own face changes. Mine changed into a conversation about regeneration, and it led me to polydeoxyribonucleotide, the salmon-derived DNA fragment the aesthetics world now shortens to PDRN. I began in October. It is July now. What has happened in those nine months is there for everyone to see, and the compliments have been coming in from every direction, which is not the reason I did it, but I would be lying if I said it wasn’t lovely.
This is both what the science actually says and what the treatment has actually done.
What it is, without the biochemistry lecture
PDRN is DNA. Specifically, it is short chains of DNA fragments harvested from salmon or trout, purified until the proteins and allergens are gone and only the polynucleotide chains remain. Salmon, because their DNA is close enough to ours that our cells recognise the material and get to work with it.
Two things then happen. The fragments switch on a receptor that calms inflammation down and encourages fresh blood supply to the area. And they hand your fibroblasts, the cells whose entire job is building collagen, a ready-made supply of the raw material they would otherwise have to manufacture from scratch. Think of it as calling in reinforcements and then delivering the bricks to the site. The result, over weeks rather than overnight, is new collagen, better elasticity and skin that repairs itself faster than it did before. That is the whole story, and it is why PDRN has become the natural companion to lasers, peels, microneedling and radiofrequency.
Does it hold up in a proper trial? Mostly, yes
Here is where honesty serves you better than enthusiasm.
The trial worth knowing about is a randomised, double-blind, split-face study by Kim, Lee and colleagues, published in the Journal of Dermatological Treatment in 2022. Twenty-seven people had a polynucleotide injection on one side of the eye area and non-crosslinked hyaluronic acid, the incumbent gold standard, on the other. Same face, same day, three sessions, two weeks apart, and neither the patients nor the assessors knew which side was which. That design is the point: it removes wishful thinking from the equation entirely.
The overall aesthetic scores came out roughly level between the two sides. But on the measures that describe skin quality rather than volume, that is roughness, pore volume, elasticity and hydration, the polynucleotide side improved more. No serious adverse events. Read that again, because it is the finding that matters: a regenerative agent going head to head with the category leader and winning on texture, on the same face, under blinded conditions.
Then there is the healing evidence, which is older and firmer. A randomised controlled trial back in 2004 found PDRN-treated skin graft wounds closed in roughly half the time of untreated ones. Reviews since have consistently found it damping down the inflammatory markers that drive ageing while stimulating fibroblasts and suppressing MMP-1, the enzyme that chews through your collagen every time you step into the sun. Build more, lose less. It is not more complicated than that.
The one caveat, and I will keep saying it because the industry will not, is size. PDRN is a big molecule, and the published consensus is that it only partially crosses the outer skin barrier. Which means injected PDRN and PDRN in a bottle are not the same proposition. The bottle works, gently, over six to twelve weeks. Anybody telling you it equals a clinic session is selling, not citing.
Who does it matters more than what is in the syringe
Everything above tells you what the molecule does. It tells you nothing at all about the hands delivering it, and in this market that is the part that should occupy you.
The measure of a good practice is what it declines to do. I have sat in a consulting room and watched my own father, who had developed fat pads under his eyes and thought that while he was there he might as well have something done, be examined properly, given the reasons the result he wanted was not one that could honestly be delivered from a syringe, and sent onward to an oculoplastic surgeon. The machines were in the building. The patient was willing. There were lakhs on the table and they were left there.
That is the standard, and it is rarer than it should be. Clinical to the core. Properly qualified, properly a dermatologist, not a weekend-certificate practitioner with a treatment menu and a sales target. Somebody who knows their patients personally and guides them conservatively, and who, when they do go aggressive with a treatment, goes aggressive for a clinical reason they are qualified to name and prepared to stand behind.
We all know the other kind. The ones who will put anything into your system because there is a quick rupee in it. I have watched that industry grow up around us, and I have watched women get hurt by it.
The law now says something similar, briefly. Topical PDRN is a cosmetic, governed by the Drugs and Cosmetics Act, 1940 and the Cosmetics Rules, 2020. Injectable PDRN is not, and a CDSCO notice of 18 May 2026 confirmed that anything supplied in injectable form falls outside the definition of a cosmetic altogether, which places it under drug and device oversight and inside a licensed clinic. If you want to go strictly by law, that is the line to hold.
What nine months has done
I have had PDRN injections and PDRN microneedling on my face since October.
My skin is brighter. The texture has changed from within, not painted on, and that distinction is everything. There is a bounce and a firmness I had quietly watched slip away and had made a kind of peace with losing. The compliments have been constant, and after a while you stop attributing them to good light.
None of this is about chasing an impossible standard. I want to be clear about that, because the noise around ageing skin is punishing enough. I turned forty and decided that regenerative medicine is something we can grow into, the way we have learned to take our sleep and our gut seriously. It is care.
And when I read that polynucleotides activate fibroblasts and drive collagen synthesis, I recognise it, because it is precisely the thing I see in the mirror. The science lines up. No wonder I feel this way.
The serums: my editor’s picks
Everything above is clinical. What follows is one woman’s experience, not trial data. All three of these are sophisticated formulations, which is the reason they are on this list and most of the category is not.
One. Dr Reju-All Advanced PDRN. It takes a deliberately contrarian position on concentration, formulating at 1,200 ppm of 99 percent pure salmon DNA rather than chasing a headline number, on the argument that past a certain point the skin cannot absorb more and a bigger figure simply buys you irritation. That is a formulation decision made by people who understand absorption rather than marketing, and it shows on the skin. It is my daily driver and has been for most of these nine months.
Two. Anua PDRN. This one sits in the reliable, barrier-friendly middle of the category, built to be used every day by skin that is also being lasered, needled and generally asked to do a great deal. It layers beautifully under everything else, it never fights the rest of the routine, and it is the bottle I recommend to anyone starting out who does not want to gamble.
Three. Medicube PDRN, the viral pink one at 10,000 ppm. I ran out of my regular bottle in the Netherlands, chanced upon this in a shop, and it was genuinely brilliant in a pinch. Fast to brighten, fast to plump. It makes the high-concentration argument and makes it well, with enough supporting formulation around the DNA to justify the number on the box.
Outside the ranking, Crossbright PDRN is phenomenal on the skin and I use it happily. My only quarrel is with the bottle, whose pump is a little awkward. The formula is faultless.
How to read that ppm number without being had
Every one of these labels leads with a ppm figure, so here is what it means. Ppm is parts per million. Ten thousand ppm equals one percent. The topical studies that show meaningful change tend to use one percent and above, which makes 10,000 ppm a sensible floor to look for in a serum.
What the number does not tell you is everything else that decides whether the thing works: the purity of the DNA, its molecular weight and therefore whether it can actually get in, and the supporting cast of niacinamide, hyaluronic acid and peptides around it. Which is exactly the argument playing out on my own shelf. Medicube’s 10,000 ppm makes the high-concentration case. Dr Reju-All’s 1,200 ppm makes the optimal-concentration case, betting on purity and particle size over volume. Both can be right, because once you pass the absorption ceiling, a bigger number on the box is marketing rather than medicine. Read purity and formulation alongside the ppm. Never the ppm alone.
The honest conclusion
PDRN is not a miracle and it is not a hoax, which in a beauty market addicted to both extremes makes it unusually trustworthy. The injectable evidence is genuinely good. The topical evidence is promising and should be held to a humbler claim. For me, at forty, it has been one of the kindest things I have done for my own skin.
The polynucleotides are doing their work. And I am, without apology, loving it.
